| The primary objective of the CHSP was to improve the health status of populations in target communities with a special focus on mothers and their under-five children. The project incorporated a baseline study at the initial stage and a Performance Monitoring and Evaluation Plan (PMP) was designed for effective periodic monitoring of activities being carried out. An evaluation was undertaken to ascertain the impact of the project on the beneficiaries. The project was a community driven, community owned primary health project in 46 communities, 34 of them in the Niger Delta. |
